What is the equation between the consequences and the human error of
this type? Since lives are irreplaceable after death the equation can
never be satisfied. Thus having another safety level is imperative. It
may be in the form of a locking system between two signalling operators
who can operate the system. Simple.
Employment will be doubled and lives saved!

Perfect designs are those which allow you to design around the Unknowable. Nuclear programmes, dams and in fact modern civilisation transgress rudely this principle driven by greed.
